Forms support text, single and multiple choices, three-choice questions, numbers, odometer readings, dates, photos and signatures. Choice answers can have different operational outcomes and photo requirements.
Drivers open the applicable published form in the driver app. They can answer questions, enter readings, add notes and capture or attach photos. The submitted check identifies the vehicle, driver, form version and outcome.
The form distinguishes a minor defect from an unsafe condition. Its critical-defect policy determines whether the result blocks the trip. The fleet team can review the submitted answers and evidence to decide the response.
